There are no limits on use or how much you can save.

"Am I assigned a dentist, or can I choose from anyone in the network?"
With Dental Benefits Plus, you get to choose from any of our participating dentists or specialists - no referral is necessary - and you can switch dentists at any time.1
"Are there any claim forms?"
Not one.
"When do benefits start?"
Your benefits begin as soon as you receive your card. There are no waiting periods.2
"Do I have to sign up for a year or more?"
No.  Use it as long as you like.  Cancel at any time.
"What about prior conditions?"
There are no exclusions for prior conditions.
"Is Dental Benefits Plus available away from home?"
Yes, Dental Benefits Plus is a nationwide program.4
"Why doesn't everyone join?"

will be assigned to you based on your location.

2 Waiting period refers to the amount of time between the start of your benefits (i.e. your effective date) and the availability of the benefits for use. It does not refer to the length of time between the time you enroll and the time you are active (i.e. effective date).
