  Dental Benefits Plus is a product of Benefit Services of America

  The dental network is provided by the Signature Dental Network. All participating providers are independent contractors, not employees of the plan or the Signature Dental Network, and are solely responsible for the provision and quality of professional services.
  Details of the terms and conditions governing the use and benefits included with membership in Dental Benefits Plus are contained in the membership enrollment package. 
  Dental Benefits Plus providers are subject to change.
1 For California Members with effective dates of 1/1/2004 or earlier your Plan general dentist will be assigned to you based on your location.
2 Waiting period refers to the amount of time between the start of your benefits (i.e. your effective date) and the availability of the benefits for use. It does not refer to the length of time between the time you enroll and the time you are active (i.e. effective date).
3 In Colorado and Maryland members are required to pay $1 for oral exams and bitewing x-rays. In Washington State members are required to pay $10 for oral exams and bitewing x-rays.
4 Dental Benefits Plus is not available in all areas. Specialists may not be available in all markets across the United States.
5 Savings on eye exams and LASIK surgery are available in selected markets across the United States.
6 Dental savings illustrations are based on 2004 national averages taken from the Original National Dental Advisory Service® Comprehensive Fee Report™.  Your savings may vary.
7 The hearing instrument program is not available in Texas or Colorado.
